Orthodontic Emergency Care in Berkeley, CA

An orthodontic emergency is any problem with your braces or aligners that causes pain or cannot wait, such as a broken bracket, a poking wire, or an injury to the mouth. Berkeley Family Orthodontics helps patients handle these quickly, with simple at-home steps for minor issues and prompt appointments for anything that needs the doctor. Call (510) 849-3613.

Most orthodontic problems are not true emergencies, and many can be eased at home until you can see us. Still, when a wire is poking your cheek or a bracket pops loose the night before a big day, it helps to know exactly what to do. At Berkeley Family Orthodontics, Dr. Robert Iezman and our team have guided Berkeley patients through these moments for decades. Because Dr. Iezman treats every patient personally, the orthodontist who knows your case is the one who helps you through the fix, not a rotating on-call doctor who has never seen your teeth. This page walks you through common issues, quick at-home relief, and when to call.

What Counts as an Orthodontic Emergency?

True orthodontic emergencies are rare, but they do happen. Call us right away, or seek urgent medical care, if you have:

  • Severe pain or swelling in your mouth, gums, or face
  • A direct injury to the teeth, jaw, or mouth
  • A tooth that has been knocked loose or knocked out
  • An appliance that is badly broken and cannot be managed at home

Most other issues, such as a loose bracket, a slipped band, general soreness, or a wire that has shifted, are minor. They are still worth a call during office hours so we can schedule a repair, but they can usually wait a day or two without harming your treatment.

Quick Fixes You Can Do at Home

It helps to keep a small kit on hand: orthodontic wax, dental floss, clean tweezers, a heavy-duty nail clipper, cotton swabs, a pencil with an eraser, salt, over-the-counter pain reliever (acetaminophen or ibuprofen), and a topical numbing gel (such as Orabase or Ora-Gel). With those on hand, here is how to handle the most common problems.

  • Poking or loose wire: Use a cotton swab or pencil eraser to gently push the wire flat against the tooth. If it still bothers you, cover the end with a small ball of orthodontic wax. If a wire is very long and cannot be tucked away, you may trim it with a clean nail clipper, then call us so we can check it.
  • Loose or broken O-ring or tie wire: If a small rubber ring or tie wire slips off, you can often reposition it with clean tweezers, or remove a loose tie wire the same way. Cover any rough edge with wax for comfort.
  • Loose band or bracket: A bracket or band that comes loose but is still attached to the wire can usually stay in place. Cover it with wax if it rubs, and call during office hours to schedule a repair.
  • Mouth sores or irritation: Dab a little numbing gel on a cotton swab and apply it directly to the sore spot. Rinsing with warm salt water also helps.
  • General soreness after an adjustment: Mild aching for a day or two is normal. An over-the-counter pain reliever and soft foods make it easier.

These steps are for temporary relief. Please still contact our office so we can make sure your appliance is working the way it should.

Injuries to the Mouth or Teeth

If you take a blow to the mouth or teeth, whether or not you wear braces, apply ice to reduce swelling and contact your dentist as soon as possible, since an X-ray may be needed. If a tooth is knocked out, place it in a container of milk or saline and get to a dentist or oral surgeon right away, because quick action gives the best chance of saving it. If braces or an appliance are damaged in the injury, call us after your dental visit and we will repair or replace what is needed.

After Office Hours

If a genuine orthodontic emergency causes pain that cannot wait until the next business day, call our office at (510) 849-3613. Our message will explain how to reach us for urgent help. For your safety and to give every patient timely care, we are not able to accept walk-in emergencies, so please call first.

Frequently Asked Questions About Orthodontic Emergencies

Yes. If you are far from Berkeley and have a true emergency, any local orthodontist can provide temporary help. For non-urgent issues, the at-home steps above will usually hold you over until you are back and can see Dr. Iezman, who knows your treatment plan.

A single loose bracket rarely sets your treatment back if it is repaired promptly. Keep it covered with wax for comfort and call us during office hours to schedule a quick fix.

Almost never. A broken or poking wire is an orthodontic issue, not a medical one, and the at-home steps above give relief until we can see you. Reserve the ER for serious facial trauma, uncontrolled bleeding, or trouble breathing or swallowing.

Tuck the wire flat with a pencil eraser and cover the end with orthodontic wax. If you are out of wax, a small piece of sugarless gum can work in a pinch until morning.
